Housing Case Manager
The Housing Case Manager (SHCM) skillfully provides strengths-based support to persons experiencing housing instability and assists with all aspects of compliance with OMH, HUD and other contract and regulatory requirements, providing effective service delivery that complies with contract and regulatory requirements and meets or exceeds the program’s targeted outcomes. The SHCM will have a specified participants’ caseload and ensure coordination of participant’s needs, bringing expertise in one or more of the following areas: mental health, homelessness, addictions, dual diagnosis and or domestic violence.

The Housing Case manager’s (HCM) core function is the coordination of care for resident services. The HCM will provide education, support, and accountability.

Housing Solutions Specialist - Schuyler
Core function is to develop a financial plan to assist those at-risk, in retaining their present housing and any arrear payments for eviction prevention. The HSS will also assist those who are homeless in rapidly regaining housing. The HSS will identify and/or secure housing subsidies going forward in accordance with the funding sources' regulations.

Housing Support Specialist
Provide continuity of service through hands-on instruction/assistance to participants, as needed in areas intended to promote housing stability and independence. Ensure the safety and well-being of all participants. Assist and support the duties, tasks, and responsibilities of the Senior Housing Case Manager as directed, as well as during his/her absence.
